CITIZEN COMMUNITY REHABILITATION SERVICE AWARD
(Not employed in Rehabilitation Field) Same criteria as Jacqueline Hanback Award.
ORGANIZATION REHABILITATION SERVICE AWARD
(For Industry, Business, or Agency Preferably Outside of the Formal Rehabilitation Field - But Not an Absolute Condition) Recognition for (1) Commitment to the hiring of employees with disabilities, (2) development and provision of support services within the organization for employees with disabilities, (3) efforts made to train and promote employees with disabilities.
EXCELLENCE IN MEDIA AWARD
Presented to the Media Profession (actor, director, producer, writer, network, reporter, etc.) or to a person whose media Production television program, film, commercial, radio broadcast, printed material). in the past year (or few years) has made a noticeable media contribution toward the advancement of persons with disabilities.

Special Statewide Awards
MARELENE A. NELSON SERVICE AWARD
To recognize outstanding services or contributions to the Illinois Rehabilitation Association. Nominee must: 1) be an IRA member, 2) have actively participated in IRA for at least five years at the Chapter, Division and/or State level (including the Statewide conference), 3) have participated within the National Rehabilitation Association, and 4) have promoted IRA awareness in the community, State and national levels.
LORENZ/BAKER OUTSTANDING SERVICE AWARD
To identify an emerging leader who has made or is making significant contributions in developing services for people with disabilities. Nominees will be evaluated based on wether they have: 1) had one to ten years of professional service within the field of rehabilitation, 2) introduced or used innovative services, 3) demonstrated leadership in the field of rehabilitation, 4) participated in volunteer activities, 5) participated in research in the field of rehabilitation, 6) published relevant articles, 7) participated in relevant organizations at a national, state, or local level.
If you would like to make a nomination for either of the above awards, please write a nomination letter which includes (1) the name, title, organization, address and phone number of the nominee, (2) the name of the award you are nominating them for, (3) a description of the nominee’s accomplishments which has caused you to nominate them, and (4) your name, title, organization, address and phone number. In addition, it is extemely important that you send along as many letters of support as possible which substantiate your nomination.

A TRIBUTE TO JACKIE HANBACK
Jacqueline T. Hanback had been known to DORS for over 25 years. She began her employment with the agency in September 1963, as a Rehabilitation Counselor assigned to Macoupin County. She later was assigned to a psychiatric caseload working with patients at the Jacksonville State Hospital. Her last counselor assignment was to a general caseload in Jersey and Greene Counties. Even though Jackie left her position as a counselor, she continued to furnish support to many of her former clients until the time of her death.
Jackie transferred to the Central Office where she was a Psychiatric Consultant for the State. While there, she was later assigned to the Quality Assurance Unit working with Jack Collins and John Craig. In October 1980, she returned to the Jacksonville Office as Rehabilitation Services Supervisor for the Jacksonville & Quincy offices. When that supervisory area was split, Jackie continued as supervisor in Jacksonville.
As a very caring individual interested in rehabilitation, she was involved in many professional organizations. Her involvement in NRA/IRA included Illinois Director (6668), representative (6869), Regional Membership Chair (6667) and National Representative (7477). She was also President of the Central Chapter. As a member of NRCA, she was Membership Chair (7071), Secretary/Treasureer (7273), Board Member at Large (7477), Program Chairman (75) and Licensing Task Force (76). Her work in IRCA included Secretary (66), President (69) and NRCA Representative (7477). Jackie was a member of IWA District VI serving as ViceChair (7071) and Chair (7273). She was a member of APGA, ARCA, serving on Research Awards Committee (72), Chair (7880); Certification Maintenance committee, Professional Advisory committee, Accreditation Council of Psychiatric Facilities, Joint Commission on Accreditation of Hospitals (71), Illinois Alchoholism Counselor Certification Task Force (7779), Director's Forum (8687), Liaison to National Organization on Disabilities (8789), Education Commitee of Chamber of Commerce (8789), Passavant Hospital Board (8688), Youth Attention Center Board (8889) and ELm City Center Board.
In addition to her professional affiliations, she was active in community service. She was a member of the Scott County Historical Society, Soroptomist Business Woman's Club, Jacksonville Art Association, National and Illinois Art Education Assn., Adams County Diabetes Assn., Scott County Community Choir, Winchester School Board and was a Passavant Hospital volunteer. Jackie was registered with Who's Who of American Women, Who's Who in the Midwest, and Who's Who in America. She was named "Boss of the Year" by the American Business Women's Assn. in 1981.
Prior to her employment with DORS, Jackie was employed as Work Supervisor at ELm City Center and an art teacher in School District #117. Jackie was the daughter of James and Mary Tankersley and was married to William Hanback, Director of Illinois Rural ELectric, who preceeded her in death in 1977. She leaves behind a daughter Mary Murla McDonald of Quincy, a grand daughter Allison, a sister Blanche of Steedman, MO and a brother Jimmie of Galveston, TX Jackie attended Jacksonville High School, MacMurray College graduating with a Liberal Arts Degree and University of Illinois with a Master's Degree in Counseling and Guidance. She enjoyed jazz music, chinese cooking, art, traveling and entertaining guests in her home.
Even though Jackie's title was supervisor, she was a personal friend and confidante to her coworkers. She always went the extra mile for them, both professionally and personally.
